After receiving a diagnosis such as ALS navigating the services that you are entitled to from the Veterans Administration becomes more important than ever. Our ALS Clinic provides not only medical management of your symptoms, but assists with adaptive equipment and navigating the VA system to maximize your benefits.

The goal of the ALS system of care is to provide a comprehensive assessment of all of your medical and personal needs to maximize health, safety, function and quality of life.


If you are a resident of Rhode Island it is strongly encouraged that you complete this form and return to the RI Emergency Management Agency so that you will be placed on the registry in the event of a disaster that would cause loss of power or telephone.  Having your name on this registry will place you on a priority list to have your services turned on as soon as possible.
