Nephrology and Primary Care Social Work Coordinator

The Nephrology Coordinator provides on-going supportive counseling, education, and case management to patients with End Stage Renal Disease and to those Veterans who are receiving kidney replacement therapies at Providence VAMC, and to their families. Transition and Case Management (TCM) Program Coordinator

The TCM team at the Providence VAMC outreaches to all post-9/11 Veterans in Rhode Island, Southeastern MA, and parts of Connecticut.  The team provides a seamless transition into VA healthcare, assistance with VBA claims and educational opportunities, offers support, case management services, and referral to appropriate community resources to help make their adjustment from the military to civilian life easier.

Primary Care
Open Access Telephone: 401-273-7100 x 4229 Social workers in the Primary Care Clinic assist with a broad range of psycho-social needs for Veterans and families accessing primary care. They provide information about resources, such as advance care planning, utility and food assistance, home care, respite care, nursing homes, cognitive screenings, transportation and supportive counseling.

Caregiver Support Program Coordinator

The Caregiver Support Program is comprised of two tracts: a General Caregiver Program and the Program of Comprehensive Assistance for Family Caregivers. The program team includes a Social Worker and a Behavioral Health RN, who are available to provide support to Caregivers of Veterans in Rhode Island, Southeastern Mass. and parts of Connecticut. The team offers assistance in navigating VA, to include VA health care, benefits counseling and VBA claims. In addition, outreach, home visits , education, resources and case management services are available, to include referral to appropriate community resources.

The IPVAP is committed to ensuring that Veterans, their partners and VA employees who experience or use IPV are treated with dignity and respect. Veterans will be screened, assessed and offered services including crisis assistance, safety planning, advocacy, case management, counseling and/or housing.
