The OEF/OIF/OND team at the Providence VAMC outreaches to all active duty service members, returning combat Veterans, and their families in Rhode Island, Southeastern MA, and parts of Connecticut.  The team provides a seamless transition into VA healthcare, assistance with VBA claims and educational opportunities, offers support, case management services, and referral to appropriate community resources to help make their adjustment from the military to civilian life easier.

TBI/Polytrauma/Spinal Cord Injury Coordinator

Primary Care
Tonya Maselli-Mcconnell, LICSW

Social workers in the primary care unit assist with psychosocial needs, advance directives, and caregiver supports. They provide information about Veteran and community resources.  A few such resources include information about housing, fuel assistance, employment agencies, food stamp programs, home care resources, transportation, and  guidance with Veterans benefits.  Social Workers are also available for supportive counseling to Veterans and their families. 

Geriatrics and Extended Care (GEC)

Home Health Aid Program (HHA)

The HHA program works to assist in providing personal care and related support services that enable frail and disable veterans to live at home.

Home-Based Primary Care (HBPC)

Provides home-based primary care services to eligible veterans.

Palliative Care

The palliative care social worker is a member of palliative care team which provides support, education and care planning to veterans and their families with advance illness

Coordinator Nursing Home & State Veterans Home Coordinator

The community Nursing Home Program provides administrative oversight to several nursing homes in RI and southeastern Massachusetts.  A social worker or nurse alternate monthly follow-up visits to assure that veterans receive timely, appropriate, and quality care.

Inpatient Psychiatric Unit

Social workers on the in-patient psychiatric unit complete comprehensive psycho-social assessments, provide supportive counseling and education to patients and families, and coordinate safe and appropriate discharge plans.  (Guide to Mental Health Services) 

Caregiver Support Coordinator
